depends on the person, depends on the situation, and depends on how sure I am of myself. e-mail/letter is a document that can be kept forever so it's the most risky. Phone & in person are more 'professional' and also less risky that it'll be used against you in the future (unless its your girlfriend where everything is remembered and stored like a giant book :-p).

have to confront someone in person. otherwise the negative part of confronting someone can be misunderstood, or you cant hug them after ;)
you can confront over skype/video chat though if you know the person well enough :)
